Parker Navigator


Parker Navigator is a Senior Care Coordination program committed to helping seniors in Middlesex County. Clients of this program are assigned a Care Navigator, a healthcare professional who is knowledgeable of aging services. The Care Navigator meets with each client to learn more about their specific needs and works with them to create a plan to age well at home. 

The Care Navigator can help you with:
  • Knowing what you need to age well
  • Making a plan to age well at home
  • Finding senior services
  • Making appointments
  • Applying for entitlement programs
  • Getting education and support

Introducing your Care Navigator: Megan Kovaltchouk, MPH, LNHA
Megan has 14 years of experience working in aging services. She has spent the last 10 years working at Parker in a variety of roles, including Nursing Home Administrator and Senior Director of Home and Community-Based Services. Megan earned a Masters of Public Health from Rutgers University. She holds a Certificate in Case Management from Boston University and is a Certified Dementia Practitioner. She is also an Aging Life Care Professional® and a member of the Aging Life Care Association®. Megan enjoys helping clients to set and achieve their wellness goals. She looks forward to learning more about you!
