Post-Acute Rehab

Therapist and Resident in Rehabilitation Center

If you’ve recently been hospitalized for an illness, injury, or surgery, you will most likely need post-acute rehabilitation services to help you safely transition from the hospital to your home. You also need a team of therapists who will customize your therapy program to best suit your needs, and guide you through each exercise with compassion and patience to help you reach your goals.

We offer comprehensive post-acute rehabilitation, with round-the-clock skilled and sub-acute nursing services at Parker at Somerset (formerly known as Parker at McCarrick,) our long-term care residence located in Somerset, New Jersey. Parker at Somerset was honored with a Five-Star rating from the Center for Medicare and Medicaid Services (CMS), in all categories, which places the community as one of the top-rated nursing homes in the state. Less than 10 percent of New Jersey nursing homes achieved a deficiency-free survey over the past year.

Our clinical programs are overseen by an administrator and onsite geriatric physician.
